AFFIDAVIT

I, Maria Ligaya, Filipino, of legal age, and resident of 123 Bagong Liwanag, Sinilangan, Quezon City, Philippines, having been duly sworn in accordance with law, hereby depose and state:

1. That I have personally known MARY ANNE for a long period of time having been her classmate and friend from Elementary to College;

2. That I know for a fact that the person by the name of "MARY ANNE" appearing in some documents and records specifically in her Birth Certificate and the name "MARY ANN" as appearing in the records of the Philippine Health Insurance Corporation's system refer to one and the same person and that her true and correct name is "MARY ANNE"; and 

3. As such, I execute this Affidavit or attest to the truth of the foregoing facts and for whatever legal purpose that this Affidavit may serve.
